    • Job Details
    • Must have acute hospital experience in CM and discharge planning
    • Do not submit CM's with only SNF, REHAB or Hospice experience
    • Must have prior travel experience
    • Certification is not required
    • Must be willing to work in weekend rotation
      • Job Requirements
      • 1 yr. minimum recent experience with discharge planning as a Clinical Case Manager with a heavy focus on discharge planning in an acute care hospital setting
      • Need to have acute INPATIENT case manager experience
      • BLS
      • InterQual
      • EPIC
      • Allscripts/Careport
        • Schedule Information
        • Must be willing to work in weekend rotation
          • Unit Specific Information
          • Must have acute hospital experience in CM and discharge planning
