A company is looking for a Triage Registered Nurse (Remote).
Key Responsibilities
Conduct intervention opportunity evaluations and respond to patient
monitoring alerts
Develop and update care plans while collaborating with caregivers
and family support
Work with patients through various communication methods to achieve
chronic care management goals
Required Qualifications
Compact AND New York nursing license
Graduated from an accredited nursing program
At least 2 years of nursing experience with adult and geriatric
populations
Confidence in telephonic triage and assessment skills
Ability to work remotely with a private workspace
